The Kansas Patient Centered Medical Home Initiative (Ks PCMHI is a project of the Kansas Academy of Family Physicians in conjunction with the Kansas Association of Osteopathic Medicine, the Kansas Chapter of American Academy of Pediatrics, the Kansas Chapter of the American College of Physicians, and the Kansas Medical Society.
The Ks PCMHI provides education and information regarding the Patient Centered Medical Home (PCMH) and encourages practices to move to the PCMH model thereby improving population health and clinical outcomes. Additionally the purpose of the Ks PCMHI is to:
Officially launched in January of 2011, the initiative will span three years, concluding in December of 2013.
Return here often to see practices' progress in the pilot, to share Kansas specific PCMH information and stories, and to learn more about the improved quality of care that PCMHs offer.
Kansas’ primary care physicians organizations and health-related foundations are looking at ways to help providers implement this important and new model: the Patient Centered Medical Home.
