The Kansas Patient Centered Medical Home Initiative is pursuing the TransforMED model of a PCMH, a partner in the initiative that is a subsidiary of American Academy of Family Physicians. Kansas’ primary care physicians organizations and health-related foundations are looking at ways to help providers implement this important and new model.
Funding for the Kansas PCMH Initiative is provided by the Kansas Health Foundation, United Methodist Health Ministry Fund and the Sunflower Foundation. Additional funding was received from the Kansas Department of Health and Environment’s System in Sync program.
The initiative is administered through the Kansas Academy of Family Physicians.
