Hostetter Award Nomination

The KAFP-Foundation announces that nominations are now open for 2019 KAFP-Foundation Hostetter Awards. FMIG faculty sponsors (only) are invited to nominate fourth year medical students from their campus who have promoted the values of family medicine during their medical school tenure.  2006 was the inaugural year for the award which was originally funded by a gift from the estate of Phillip Hostetter, MD, long-time member, former president, and family physician. The KAFP-Foundation (KAFP-F) now funds the award in his honor.

Nomination Process

  1. Nomination Form: A completed nomination form for each nominee is required.  We request up to 2 nominations from each campus. Only nominations by FMIG faculty sponsors are accepted.
  2. Nomination Letters: The faculty sponsors of the FMIG on each campus are invited to nominate students they believe are suitable for the award. Each letter may be up to 2 pages in length. A letter of recommendation for residency is satisfactory to include as a nomination letter.
  3. CV: A copy of the students’ CV is required.
  4. Supporting Letters: Others may write letters of support for nominees, not to exceed 4 pages total.
  5. Due Date: Nominations are to be received by KAFP-F by March 22, 2019 for the 2019 KAFP-Foundation Hostetter Award nominations.

The award is designed to recognize senior medical students going into family medicine who have promoted the values of family medicine during their years in medical school. Specific examples of such activities will enhance nominations.

Selection: The KAFP-F Board of Trustees will select up to 1 student from each campus (total of 3 students) as recipients.

Expectations: The KAFP-F expects recipients to attend the KAFP Annual Meeting Member Meeting Lunch June 15, 2019 to receive the award.

Award amount: KAFP-F will provide a check for $500 and a certificate or plaque to the awardees at the Annual Meeting.
