KAFP requires a Speaker Agreement & COI to be completed by each speaker at Fam Med Forward.

If a speaker is giving multiple presentations, they must complete this form for each presentation as the form requests specifics for each presentation.

Items to have ready to complete your Speaker Agreement:

• Title of your presentation.  Shorter titles, 5 words or less, are preferred.
• Learning Objectives (LO) for your presentation, please provide up to three objectives that complete this statement: “At the conclusion of this session, attendees should be able to…”
• Short biography* (not a CV)
• Head shot*

*Biography and head shot will be published in meeting materials.
