This will be emailed to partners upon completion of CME Partnership Agreement &amp; Deliverables form.<\/li>\n<li aria-level=\"1\">CME Statement. To be placed in marketing materials, agendas, websites. This will be emailed to partners upon AAFP CME approval. A pending CME statement will be provided upon CME application being submitted to AAFP.<\/li>\n<li aria-level=\"1\">CME Certificate of Participation Template. Due back to KAFP before the start of the program.<\/li>\n<li aria-level=\"1\">Handouts. Due to KAFP prior to the start of the program.<\/li>\n<li aria-level=\"1\">AAFP accreditation statements\/seal as listed on the program website (screenshot).<\/li>\n<li aria-level=\"1\">Activity planning documentation, including how the following were identified\/defined:\u00a0 Professional practice gaps, needs assessments, core competencies, learning objectives, and learner engagement.<\/li>\n<li aria-level=\"1\">Learner Evaluations. Scanned and sent to KAFP two weeks post event.<\/li>\n<li aria-level=\"1\">Description of the faculty selection process.<\/li>\n<li aria-level=\"1\">Completed Conflict of Interest forms.
