Photo Release Statement
Kansas Academy of Family Physicians may take photographs and/or record audio and video at organized KAFP functions/events/activities. By attending, you consent to the use of any photographs, audio, and video recordings of you by KAFP and its designees in KAFP communications and promotions, or for any other lawful purpose.
Cancellations and Course Refunds
Written requests for cancellation of a meeting registration received no later than four weeks before the first day of the meeting will be refunded less an administrative fee, determined per event. Cancellations or no-shows after that time are not eligible for a refund.

Conflict of Interest/ COI Disclosure
All KAFP Board of Directors, KAFP Foundation Board of Trustees, Committee members and Presenters are required to complete a COI form annually or per educational event.

Travel Reimbursement
KAFP Board of Directors please use the Travel Reimbursement for Board of Directors form to submit any Board travel reimbursement requests. Expenses incurred are reimbursed as outlined in the budget the board approves annually. Please email us at info@kafponline.org if you have questions about items that can be submitted for reimbursement.

Other Corrections or Refunds

Please contact the KAFP offices, info@kafponline.org or 913-214-1117, if you feel your credit card has been charged incorrectly. These situations will be handled on an individual basis.
