Chronic Disease Self-Management leader training available for you & staff

Leader Training is available for you and/or staff for Chronic Disease-Self Management and Diabetes Self- Management. These trainings are in Dodge City, Kansas during the month of May. For more information and please download and share the Tools for Better Health flyer. There are several ways you can promote self-management education in Kansas.  Anyone can become certified through attending training for four days. Participants will receive everything needed to facilitate a workshop, including training, support, books, CDs, and workshop supplies at no cost. Become a partner by referring your patients to self-management workshops. Assistance will be provided to streamline a referral process.
