Want to improve population health and understand how social determinants affect your patients? Join the PRIME SAN team for an exciting webinar to introduce you to the Population Health Assessment Engine (PHATE). Developed collaboratively by the American Board of Family Medicine and the University of Missouri, PHATE allows users to view the communities they are serving, community data, community partners, and disease and (poor) quality hot spots. To help you train others to use PHATE, the Robert Graham Center and HealthLandscape have developed educational materials that teach clinicians, administrators, and learners how to incorporate PHATE and population health concepts into practice. This webinar is free of charge.
